Statements of Uzbek citizens residing abroad are considered consul. Statement surname, name and patronymic of execution filed by the applicant on his 16 years ( turnout applicant is required ) .
Applications for change of surname , name and patronymic are considered in the case of :
if the first name and patronymic are discordant ;
if the first name and patronymic not correspond nationality ;
when returning optional premarital surnames ( while in the marriage and after divorce );
when a desire to elect a common name for a family ( if the marriage did not take wife common surname );
on other grounds , if the consul found them respectful .
Documents required for a change of surname, name and patronymic
Statement surname, name and patronymic (installed application form the consul ) .
The birth certificate of the applicant.
If the applicant is married , the marriage certificate , if divorced - divorce certificate .
If the applicant has minor children , the children's birth certificates .
Extract from the passport or other identity document of the applicant.
If the applicant is a minor (16 to 18 years) or adjudged incompetent , the request of the parents or guardian or custodian .

For persons who have not attained the age of majority (16 to 18) must also be written parental consent .
Payment for consular services and reimbursement of actual expenses.
If the person is over 16 years old, submit an application for change of surname , name and patronymic to obtain a passport or an equivalent identity document, the application shall be accompanied by a birth certificate and a copy of the birth certificate .
