Documents required to get the service

• A copy of the passport (of the person transferred under guardianship, the guardian);


• Copy of birth certificate (of the person transferred under guardianship, the guardian);


• Copy of the judgment;


• Copy of certificate of disability;


• Copy of the certificate with a family can cope with health clinic on (the person transferred under guardianship, the guardian);


• A copy of the certificate from the psychiatric clinic ward person;


• A copy of the certificate from the STI clinic ward person;


• Copy of the certificate with the tuberculosis hospital ward person;


• Copy of the certificate with drug treatment clinic ward person;


• A copy of the certificate from the body of social protection (of the person transferred under guardianship, the guardian);


• A copy of the certificate from the residence showing all prescription (f-17) (of the person transferred under guardianship, the guardian);


• A copy of the survey apartments ward person issued with the district health department;


• A copy of the plan of the house / apartment (of the person transferred under guardianship, the guardian);


• A copy of the death certificate of the parent entity transferred to custody;


• A copy of the passport of family members guardian;


• A copy of the notarized consent for the issue of guardianship family guardian.

Free of charge.

Period of dealing with an application

15 to 30 working days.

Methods for obtaining results

In electronic form. If desired, the applicant can obtain a paper copy of the decision of the Khokim.

Result of the service

Getting electronically or in paper form a copy of the decision or refusal khokim prenyatie solutions.

Reasons for refusal

• Failure to provide the documents provided services regulation;


• The unreliability of the documents provided;


• Application containing not normative lexicon, and abusive language, as well as any advertising;


• Any other action contrary to law.

Order of appeal

In case of refusal the applicant may appeal to a higher authority in the prescribed manner.
