Documents required to get the service
- Medical child's birth certificate.
- Passports (must be scanned all the pages) parents or equivalent documents.
- Stamp marriage certificate or a declaration of paternity.
Legal basis for service provision
- Family Code of the Republic of Uzbekistan.
- Law of the Republic of Uzbekistan "On electronic digital signature".
- Resolution of the Cabinet of Ministers "On approval of the necessary regulations for the implementation of the Family Code of the Republic of Uzbekistan" dated 12.04.1999 of №171.
- Order of the Minister of Justice of the Republic of Uzbekistan "On approval of the instruction on the procedure of record keeping in the bodies of civil registration" dated 31.12.2003 of №400.
Cost and method of payment for the service
According to the resolution of the Cabinet of the Republic of Uzbekistan "About the statement of forms of stamp certificates and the size of the stamp duty for their delivery" dated 25.04.2012 of №119, the stamp duty for issue of the stamp certificate on a birth makes 15 percent of the minimum wage established in the republic. Payment is made by personal handling.

Methods for obtaining results
A notification is forwarded to the user's personal account on the Single portal and/or emailed to the user's email address specified during registration on the portal.
Result of the service
Receiving notification of the time and place of reception, out of turn, head of the department of the CR of Sergeli district.
After receiving the notification, the applicant personally going to the department of the CR of Sergeli district with the original documents and pay the fee.
Reasons for refusal
- There is no registration of both parents in the territory of Sergeli or the child was not born in the territory Sergeli district.
- Absence of the necessary documents.
Order of appeal
The applicant has the right to appeal the quality of the service and action (inaction) of the official institution in the order established by law.
